Complications of Untreated Varicose Veins and Venous Insufficiency: Swelling of the Ankles/legs and lymphedema

Leg swelling (swelling is also known as edema), is a common complaint related to varicose veins and underlying venous insufficiency. Leg swelling can be present simply from underlying venous insufficiency without visible signs of varicose veins or spider veins. As a result, a venous insufficiency study by duplex ultrasound scanning is part of the recommended work-up for leg swelling. 

The Duplex Ultrasound involves using high frequency sound waves to look at the speed of blood flow, and structure of the blood vessels. The term “duplex” refers to the fact that two modes of ultrasound are used, Doppler and B-mode. The B-mode transducer obtains an image of the vessel being studied. The Doppler probe within the transducer evaluates the velocity and direction of blood flow in the vessel.

The swelling usually affects the ankles. Most people with vein-related swelling experience progressive swelling throughout the day, that is worse at night time and improved in the morning (they usually wake up with normal appearing legs in the morning but by night-time, shoes may feel tight.) Vein-related swelling is worse at the end of the day, with prolonged standing, sitting or with air travel, heat and menstruation. The degree of swelling varies from one person to another.

Leg swelling is a common symptom of varicose veins and venous insufficiency. The legs often feel heavy. It is common to notice sock lines around the ankles by the end of the day.

Compression stockings and leg elevation will help reduce swelling. If the swelling can be attributed to the varicose veins or venous insufficiency, correction of the underlying vein problem will reduce swelling.

Both Wendy Williams and Kathy Bates Suffer from Lymphedema.

This week’s Wendy Williams Show host, Wendy Williams discussed her struggles with lymphedema with guest, Kathy Bates who also has lymphedema. They both agreed that lymphedema is not given enough attention in medical education and in the medical community. We agree. Our medical speciality and society The American Vein & Lymphatic Society (AVLS) specializes in both vein and lymphatic disorders.

Lymphedema refers to swelling that generally occurs in one of your arms or legs. Sometimes both arms or both legs swell.  Lymphedema is most commonly caused by the removal of or damage to your lymph nodes as a part of cancer treatment. It results from a blockage in your lymphatic system, which is part of your immune system. The blockage prevents lymph fluid from draining well, and the fluid buildup leads to swelling. In our field, we also see phlebolymphedema, which is lymphedema secondary to chronic venous insufficiency. Venous hypertension can overload the lymphatic transport system, slowing fluid removal and causing subsequent lymphatic edema, in addition to venous edema. The result is swelling in the legs that is greater than venous edema.

n 2016, Kathy Bates was a speaker at our annual congress.   The American Vein & Lymphatic Society (AVLS) hosted what has become the largest and most comprehensive meeting dedicated to venous and lymphatic disorders in the U.S. Bates outspokenly discussed surviving and managing lymphedema after she had breast cancer and underwent a double mastectomy.

Lymphedema is not cured but can be managed. In our clinic, we may refer to one of our trusted lymphedema therapists for manual lymphatic drainage and prescribe compression therapy. We also offer lymphatic pump therapy in the office. It is also important to evaluate the deep and superficial venous system prior to initiating therapy.

Highlights of the National Vein Congress

The La Jolla Vein Care doctors and staff attended the American College of Phlebology (ACP) Annual Conference in Phoenix last week.  More than 1,000 vein care practitioners from the United States and around the world attended the meeting to share information and research in the field of vein disorders (also known as phlebology).  The speakers included vein and vascular experts from around the world, include La Jolla Vein Care’s Dr. Fronek.  Dr. Bunke was featured in the daily congress newsletter as being the first physician to complete a specialized Fellowship program in the field of Phlebology.

La Jolla Vein Care’s medical assistants and vascular sonographers attended also to learn the most update information on a variety of vein topics including; complications in sclerotherapy, the swollen limb, thrombophilia, new anticoagulants, venous stents, compression therapy, vascular ultrasound, foam and liquid sclerotherapy, vascular malformations, lymphedema, venous system during pregnancy, venous skin changes, dvt and superficial thrombophlebitis, chronic venous insufficiency and research on specific vascular topics.
